Crew Evaluation Form

Please note: only Professors can assign the grades for student taking TA 191, Activities Projects in Theatre Production. This form is a device for the crew heads, Assistant Stage Managers and the Stage Manager to have the opportunity to submit formal evaluations of the performance of each crewmember. The information is to be kept on file for the following semester in the case that grade evaluations are challenged.
Student are expected to arrive on-time for every call, perform their assignment to the best of their ability and the stay until released by their supervisor. Their action should contribute to the goals of the production. These include: 1.) Gain experience with the production of live theatre. 2.) Work collaboratively with other technicians, actors and staff. 3.) Perform the task assigned to them for the benefit of the performance and the audience.

The following check list can be used to assist with the evaluation process.
